- The subject of this public order is the purchase of a service, which for the purposes of this VZMR means the purchase of meal vouchers for the employees of the Military Veterinary Institute Hlučin, including the cost centers Chotyně and Česke Budějovice for the period from 01.02.2024 to 31.01.2026 - competitive bidding on the amount of the commission , nominal valuethe value of the meal voucher is CZK 130.00.
Small-scale public contract (hereinafter referred to as VZMR).
Subject and definition of contract performance:
This is a small-scale public contract (hereinafter referred to as VZMR) for the purchase of a service - securing the purchase of meal vouchers for employees of VVetU Hlučin, including costseditions of Chotyně and Česke Budějovice for the period from 1 February 2024 to 31 January 2026 - the nominal value of the meal voucher is CZK 130.00 - see attachment no. 2.
In total, the provider will deliver a maximum of 30,768 meal vouchers during the term of the contract, while the number of meal vouchers will not exceed 1,28 per month2 pcs.
We require the determination of the amount of the commission for the sale of meal vouchers. The value of the commission amount will be indicated to two (2) decimal places with a value higher than 0. The stated negative price by the bidder will be evaluated as a violation of the requirements of the contracting authority for the processing of the bid price.
The fulfillment will be carried outaccording to the requirements of the contracting authority on the basis of a service security agreement concluded with the winning bidder for the period from 1/2/2024 - 31/1/2026. The provider will ensure individual partial fulfillment by continuous delivery of ordered meal vouchers at the specified time and in the agreed manner - see. appendix no. 3 - Draft smlouvy.
Requirements:
- The participant shall submit professional qualification prerequisites in the form of attachments in the VZMR (supplier's attachments) in accordance with § 77 of ZVZ 134/2016 para. 1, para. 2 p. a) extract from the commercial register and trade register.
- Furthermore, filling in the price offer in the table - see. attachment no. 2 - ATTENTION - startedit according to the instructions in Appendix No. 2.
- As Appendix No. 3, insert the negotiated and possibly modified draft of the contract, which is signed by a person authorized to act on behalf of the participant's organization.
- As Appendix No. 4, insert the supplier's sworn statement
As part of the offer, we require you to supply current overview of catering establishmentsin the municipalities and cities of Hlučin, Chotyn (Hradek nad Nisou) and Česke Budějovice, where the voucher can be used for meals provided that have the character of a main meal.
Submit the required attachments in PDF format. Required documents proving the fulfillment of the basic qualification requirements and extracts from the business(business) register must not be older than 3 months from the date of publication of the VZMR.
