• Description:
• The subject of the public contract is the provision of rehabilitation and reconditioning activities leading to the improvement of the health status of the insured persons of the contracting authority, who are employees of companies (hereinafter referred to as the "organization"), with which the contracting authority (hereinafter also referred to as "ČPZP") and the selected supplier they will conclude tripartite agreements on the provision of a financial contribution for the payment of rehabilitation and reconditioning activities, the model of which is Annex No. 6 of the procurement documentation. Rehabilitation and reconditioning activities will be implemented in the form of rehabilitation and reconditioning stays (further also as "stay" or "rehabilitation and reconditioning care") and will be provided to insured persons of the contracting authority who meet the following conditions: a. are insured by ČPZP for the entire period of receiving rehabilitation and reconditioning care; b. do not have a registered payable a an unpaid obligation to ČPZP according to Act No. 592/1992 Coll., on premiums for public health insurance, as amended; c. participated in a general preventive inspection within the period set by Act No. 48/1997 Coll., on public health insurance and on the amendment and addition of some related laws, as amended, if it is a ČPZP insured person who asked the ČPZP to provide paid health care and the ČPZP did not provide this care within the time limits set by Act No. 48/1997 Coll., on public health insurance and on the amendment and addition of some related laws, as amended; d. are employees of the organization, or are employed in subsidiaries and other property-linked...
