This is a small public contractof the scope (hereinafter referred to as VZMR) for the performance of laboratory veterinary examinations of samples, including the training of persons for the diagnosis of Trichinella spp. in the meat submitted by the Military Veterinary Institute Hlučin and branches - Prague, Radošov, Chotyně, Jince, Polna na Šumava and the Military Veterinary Laboratoryava Hlučin. Performance will be carried out according to the requirements of the contracting authority on the basis of a concluded framework agreement (hereafter RD) with the winning participant for the period from 1 April 2024 to 31 March 2025. The provider will ensure individual partial fulfillment by continuous collection or collection of submitted samples for examination, by carrying out laboratory testsveterinary examinations, issuing protocols and sending them to the customer of the service. The list of required examinations is given in appendix number 2. Individual examinations will be carried out on the basis of the submitted application forms. Requests - orders will be submitted together with examined samples and electronicallyof the signature will be entered into ET NEN. The service provider undertakes to carry out individual laboratory veterinary examinations in accordance with accredited examination procedures. The expected amount of the framework agreement is a maximum of CZK 400,000 / for the above-mentioned period including VAT. However, the aforementioned framework nmust be achieved.
Requirement:
Participant submits in the form of attachments in the VZMR (supplier's attachments):
- professional qualification prerequisites according to § 77 Z ZVZ 134/2016 paragraph 1, paragraph 2 p.
- extract from the commercial register and trade register or incorporation document
- further filling in the price offer in the tablee - see appendix no. 2 - ATTENTION - start according to the instructions under the table in appendix no. 2.
- as appendix no. 3 insert a discussed and possibly amended draft of the framework agreement, which is signed by a person authorized to act on behalf of the participant's organization
- as appendix no. 4 insert an honorary supplier's declaration
- in the attachment sectionand submit the certificate of accreditation of the ČIA laboratories performing the laboratory examination
Submit the required attachments in PDF format. The required documents proving the fulfillment of the basic qualification requirements and extracts from the commercial (trade) register or the deed of incorporation must not be older than 3 monthsfrom the date of the announcement of the VZMR. Samples for laboratory examination must be transported via collection lines or by personal delivery to the customer's representative. Test reports together with invoices and a copy of the request form should be sent to the e-mail boxky: fakturace@army.cz and protocols to the branches of the Military Veterinary Institute in Hlučin, from where the samples were delivered. - Documents :
