- Rd - Laboratory Veterinary Examination of Animal Samples Including Training of Persons for the Diagnosis of Trichinella Spp. In Mase U Svu For the Freight Center of the Moravian Border and the Specialized Operations of the Military Veterinary Institute in Hlučin In 2024.conducting laboratory veterinary examinations of game samples and clinical samples, including training of persons for the diagnosis of Trichinella spp. in meat at SVU for the Hranice na Morava cargo center and specialized operations of the Military Veterinary Institute Hlučin. Fulfillment will be carried out according to the client's requirementson the basis of the concluded framework agreement with the winning bidder for the year 2024. The provider will ensure individual partial fulfillment by continuous collection or collection of submitted samples for examination, laboratory veterinary examination, issuance of protocols and their dispatch to the customer of the service. List of required heightsexamination is given in Appendix number 2. Individual examinations will be carried out on the basis of the submitted applications. Requests - orders will be submitted together with the examined samples and will be entered into ET NEN for electronic signature. The service provider undertakes to carry out individual laboratory testsveterinary examinations in accordance with accredited examination procedures. The expected amount of the framework agreement is a maximum of CZK 200,000 / for the year 2024. However, this amount may not be reached.§ 77 ZVZ 134/2016 paragraph 1, paragraph 2 p. a)-
- extract from the commercial register and trade register (incorporation certificate)-
- completed price offer in the table - see appendix no. 2 ATTENTION start according to the instructions under the table in appendix no. 2.
- as appendix no. 3 insert the discussed and possibly modifieda draft framework agreement signed by a person authorized to act on behalf of the participant's organization. In the attachment section, submit the certificate of accreditation of ČIA laboratories.
- as attachment No. 4, insert the supplier's sworn statement
Submit the required attachments in PDF format. Required documents proving the fulfillment of the requirementdays of qualification prerequisites and extracts from the commercial (trade) register or charter must not be older than 3 months from the date of the announcement of the VZMR.
Time and place of fulfillment
From 1.1.2024 to 31.12. 2024 according to the client's requirements.
Processing of the offer price
The offer will be processed in the Czech language.You can get the offer price by performing the valuation in the table - see appendix no. 2, further by filling in the requirements in ET NEN. Insert attachment No. 2 into the attachment part of the supplier in ET NEN.
The bid price will be the final price. Additional costs will not be possible. - Documents :
