- This is a small-scale public order (hereinafter referred to as VZMR) for the purchase of laboratory material for the performance of professional accredited examinations in the field of serology by an accredited laboratory at VVetU Hlučin.
Securing the supply of material with the aim of diagnosing infections. In order to prevent their spread, edthe State Veterinary Administration Prague Health Control Methodology, which must be followed according to the Veterinary Act No. 166/1999 Coll.
The purchase of material will be carried out according to the requirements of the contracting authority on the basis of a one-time purchase contract (KS) concluded with the winning bidder VZMR.
We require the delivery of the goods at the latestwithin 30 working days from the signing of the purchase contract. The list of required material for valuation is given in Appendix No. 2 - Price offer - valuation.
Requirement:
Participant shall submit the professional qualification prerequisites in the form of attachments in the VZMR (supplier's attachments) according to § 77 Z ZVZ 134/2016, paragraph 1, paragraph 2 p. and). extract from the commercial register and trade register. For §77, if the required letters from the given paragraphs are not listed - see mentioned above (§ 77 paragraph 1, paragraph 2 letter a), the contracting authority may exclude the participant from the VZMR.
Further filling in the price offer in the table - see. appendix no. 2 ATTENTION start according toe of the instructions under the table in Appendix No. 2. As Appendix No. 3, insert the discussed and possibly modified draft of the purchase contract, which is signed by a person authorized to act on behalf of the participant's organization.
If the VZMR participant is the exclusive supplier of the required material for the Czech Republic, he submits in the attachment part the Declaration of HonorPower of attorney to represent and distribute the given company for the Czech Republic with the statement that he is the only representative entrusted with this activity for the Czech Republic in 2023.
Submit the required attachments in PDF format. Required documents proving the fulfillment of the basic qualification requirements and extracts from the commercial and trade registerof the register must not be older than 3 months from the date of the announcement of the VZMR. appendix No. 4 - Supplier's affidavit regarding the socially responsible performance of a public contract in accordance with Resolution of the Government of the Czech Republic No. 531 of 24 July 2017, on the Rules for the Application of a Responsible Approach toawarding of public contracts and purchases of state administration and self-government.
