- This is a small-scale public order (hereinafter referred to as VZMR) for the purchase of diagnostic consumables - diagnostic ELISA kits - for OLD at VVetU Hlučin. Ensuring the supply of diagnostic consumables for the performance of professional accredited examinations in the field of serology by an accredited laboratory at VVetU Hlučin with the aim of diagnosing infections. In order to prevent their spread, the Prague State Veterinary Administration publishes the Health Control Methodology, which must be followed according to the Veterinary Act No. 166/1999 Coll.
The exclusive supplier of the required diagnostic material for the Czech Republic is sthe company TestLine Clinical Diagnostics s.r.o. This supplier is the only one that can offer specific diagnostic sets, used to identify infections using accredited methods according to the Health Control Methodology, which is published annually by the State Veterinary Administration. These diagnostic accredited methods inclethnic diagnostic sets are contained in the accreditation certificate No.: 282/2021 dated 17/05/2021, to which the individual accredited methodologies are attached. diagnostic consumables that are used at acredited methodologies (see Appendix No. 2 Price offer) from a specific supplier, namely the company TestLine Clinical Diagnostics s.r.o. (KS)with the winning bidder VZMR.
We require the delivery of the goods no later than 8 December 2023 due to the end of financial management for the year 2023. The list of required material for valuation is given in Appendix No. 2 - Price offer - valuation. (supplier's attachments) bill of ladingmeets professional qualification prerequisites according to § 77 Z ZVZ 134/2016, paragraph 1, paragraph 2 p. and). extract from the commercial register and trade register. For §77, if the required letters from the given paragraphs are not listed - see mentioned above (§ 77 paragraph 1, paragraph 2 letter a), the contracting authority may exclude the participant from the VZMRit.
Further filling in the price offer in the table - see. appendix no. 2 ATTENTION start according to the instructions under the table in appendix no. 2. As appendix no. 3, insert the discussed and possibly modified draft of the purchase contract, which will be signed by a person authorized to act on behalf of the participant's organization.
If the VZMR participant is the exclusive supplierbody of the required material for the Czech Republic, submits in the attached part the Declaration of Power of Attorney to represent and distribute the company in question for the Czech Republic with the statement that he is the only representative entrusted with this activity for the Czech Republic in 2023.
Submit the required attachments in PDF format. Required documents prooffulfilling the basic qualification requirements and extracts from the commercial and trade register must not be older than 3 months from the date of the announcement of the VZMR.
