- This is a small-scale public contract (hereafter referred to as VZMR) for the purchase of consumables - test tubes and microtubes for OLD at VVetU Hlučin as part of DZRO research with the aim of examining samples and diagnosing infections by the accredited PCR method in the field of molecular biology.
The purchase of the material will be carried outn according to the requirements of the contracting authority on the basis of a concluded one-time purchase contract (KS) with the winning bidder VZMR.
We require delivery of the goods no later than 5/12/2023 due to the end of financial management for the year 2023. The list of required material for pricing is given in Appendix No. 2-Price offer-startchanges.
Requirement:
The participant will submit professional qualification prerequisites in the form of attachments in the VZMR (supplier's attachments) in accordance with § 77 Z ZVZ 134/2016, paragraph 1, paragraph 2, p. and). extract from the commercial register and trade register. For §77, if the required letters from the given paragraphs are not listed - see listedabove (§ 77 paragraph 1, paragraph 2 letter a), the contracting authority may exclude the participant from the VZMR.
Further filling in the price offer in the table - see. Appendix No. 2 ATTENTION start pricing according to the instructions under the table in Appendix No. 2. As Appendix No. 3, insert the discussed and possibly amended draft of the purchase contract, which is signed by the person responsibleintended to act on behalf of the participant's organization.
If the VZMR participant is the exclusive supplier of the required material for the Czech Republic, he submits in the attached part the Declaration of Power of Attorney to represent and distribute the given company for the Czech Republic with the statement that he is the only representative entrusted with this activity for the Czech Republic in2023.
Submit the required attachments in PDF format. The required documents proving the fulfillment of the basic qualification prerequisites and extracts from the commercial and trade register must not be older than 3 months from the date of the announcement of the VZMR. Appendix No. 4 - Declaration of Honorof suppliers to socially responsible performance of public contracts in accordance with Resolution of the Government of the Czech Republic No. 531 of 24 July 2017, on the Rules for the Application of a Responsible Approach in the Awarding of Public Contracts and Government and Local Government Purchases.
