• The subject of the public order is the continuous supply of left atrial occlusion systems for the General Faculty Hospital in Prague (hereinafter referred to as "VFN"). - The subject of performance of the public contract (hereafter also referred to as "VZ") is also defined by the binding draft contracts, which are attached No. 2 of the procurement conditions (Annex No. 2a for part 1 of the RFP and appendix No. 2b for part 2 of the RFP), Appendix No. 8 of the procurement conditions - Item price list (Annex No. 8a for Part 1 of the RFP and Appendix No. 8b for Part 2 of the RFP) and Appendix No. 9 of the procurement conditions - Technical parameters subject of performance (Appendix No. 9a for Part 1 of the VZ and Appendix No. 9b for Part 2 of the VZ). The public contract is divided into parts: Part 1 VZ - System for occlusion of the left atrial appendage in procedures performed from sternomia Part 2 VZ - Left atrial appendage occlusion system allowing fully thoracoscopic approach - The supplier is entitled to submit an offer for one part, several parts or all parts of the public contract. - Each part of the public contract will be assessed and evaluated separately. - Unless otherwise specified further on, the terms and conditions apply for all parts of the public contract. - The quantity of the subject of performance in individual items listed in Annex No. 8 of the tender conditions - Item price list (Annex No. 8a for Part 1 of the Tender and Annex No. 8b for Part 2 of the Tender) is an indicative amount, or assumed.
